Optional Accidental Death and Dismemberment

Optional AD&D Rate = $.015 per $1,000 of Coverage

Coverage is equal to 2x your annual base pay. Divide the total coverage by $1,000 and then multiply by $.015 to get your monthly cost for Optional AD&D coverage. To get to the bi-weekly cost, further multiply your monthly cost by 12 and then divide by 26.
