Basic & Supplemental Life Insurance

Catalina provides Basic Term Life Insurance to you, your spouse and your child(ren) at no cost to you. Catalina also provides the opportunity to purchase Supplemental Life Insurance for you and your family, administered by Prudential.

1Per the IRS, any amount of group-term life insurance coverage provided under a policy carried directly or indirectly by an employer is considered a taxable fringe benefit. Therefore, this benefit is considered taxable and the imputed cost of coverage is included in employee's income and is subject to social security and Medicare taxes. The imputed cost of coverage is calculated using the IRS Premium Table in Publication 15-B.

Initial Enrollment for Supplemental life insurance

When you become eligible for Catalina benefits as a full-time employee, you have a one-time opportunity to enroll in Supplemental Life Insurance for you and/or your spouse without Evidence of Insurability (EOI) up to certain limits ($600,000 for employees and $50,000 for spouses). Employees who previously declined life insurance for themselves or their spouse will be subject to EOI for any amount elected.

Annual One-Time Increase for Supplemental Employee Life During Open Enrollment

During Open Enrollment, if you are currently enrolled in Supplemental Employee Life coverage, you may elect to increase the amount of your insurance by 1x earnings without EOI, but any amount that exceeds an increase of 1x earnings, or is above the Guarantee Issue amount, requires EOI. Opportunity does not apply to Supplemental Spouse Life.

What is Evidence of Insurability?

Evidence of Insurability (EOI) is an application process in which you provide information on the condition of your health or your dependent's health in order to be considered for certain types of insurance coverage.

If you are required to complete EOI, you will receive an email from Prudential to register online and complete the EOI application. If you do not have an email on file, you will receive a paper application mailed to your home address to complete and return to Prudential.

If it is not submitted timely, the benefit amount elected could be denied.

Optional Accidental Death & Dismemberment (AD&D)

You have the option to purchase Accidental Death and Dismemberment (AD&D) insurance at 2x your salary (up to a maximum of $500,000). AD&D provides additional benefits to help keep your family financially secure should you lose your life, sight, hearing, speech or use of your limb(s) in an accident. These benefits are paid as a percentage of your coverage amount, from 25% to 100%, depending on the type of loss.

Business Travel Accident Policy

When traveling on behalf of Catalina, be assured you are protected under a company-paid business travel accident (BTA) insurance policy should an accident occur. You are covered in the event of death or dismemberment as a result of an accident while traveling on company business. The plan does not pay benefits if the loss is the result of illness or disease.
