2025 LiveWell Medical Premium Discount FAQs

Catalina will offer a LiveWell Medical Premium Discount again for the 2025 plan year to continue to promote the importance of wellness and preventive care. We have also expanded the list of eligible preventive services that qualify for the discount!

To qualify for the 2025 LiveWell Medical Premium Discount, employees hired on or before July 1, 2024, and who are covered under one of Catalina’s medical plans, must complete two steps by October 31, 2024. If you cover a spouse or domestic partner on Catalina’s medical plan, he or she must also complete both steps by the deadline for you to be eligible for the discount.

Employees hired after, or are covered under the medical plan following, July 1, 2024, are exempt from the LiveWell discount requirements and will automatically receive the discount for 2025.

Please note: If you miss the deadline of October 31, 2024, you will not be eligible for the 2025 discount.

STEP 1: PREVENTIVE SERVICE

• Seek a preventive service between November 1, 2023 and October 31, 2024.

• Eligible preventive services include:

  • Adult Routine Physical

  • Annual Well-woman Exam

  • Mammogram

  • Colonoscopy

  • Sigmoidoscopy (new!)

  • Flu Shot (new!)

  • Immunization (new!)

  • Routine Eye Exam (new!)

  • Enroll In Aetna’s Maternity Program (new!)

  • Enroll In Aetna Health Connections Disease Management Program (new!)

STEP 2: HEALTH ASSESSMENT SURVEY

Complete or recomplete an online health assessment survey via Aetna’s website (www.aetna.com) between November 1, 2023 and October 31, 2024. Detailed instructions are available in the 2025 LiveWell Medical Premium Discount FAQs.

If you cover a spouse or domestic partner on Catalina’s medical plan, he or she must also complete both steps by the deadline for you to be eligible for the discount.
